I give MCFL permission to use the enclosed photo for the "2010 Walk Baby Contest" and understand that it becomes the property of MCFL upon submission and is non-returnable. MCFL reserves the right to edit, adapt, trademark, copyright, publish, and/or transfer to third parties all photos/submissions. I also understand and agree that the photo of the baby selected can be used for any and all promotion of the 2010 Walk. I understand and hereby give MCFL permission to use my child's name in any publication MCFL deems appropriate. All entries must be submitted for receipt by June 11, 2010 at MCFL. The rules are simple, easy to follow, and require these criteria:
1) The baby must be between 6 months and 24 months of age and MCFL must have a current photo, preferably shot by a professional studio.
2) The photo must be at least 3" x 5" in size.
3) The photo must be submitted to the MCFL office and received no later than June 11, 2010
4) You must complete the entry form in its entirety.
5) Your photo submission is non-returnable, becoming the property of MCFL, and gives permission to MCFL to use the photo in this contest.
